*I agree to make payment for my child/ward Concert Participation Fee of $180.00 per student that includes:

1) Performance costumes & accessories 

2) Make-up & props if any

3) Meals 

4) Concert Video

*I understand that the Participation Fee is confirmed after payment and NON-REFUNDABLE.

*I understand that I will receive a payment confirmation after making the payment, and a reminder on the rehearsal schedules to be given one month before event.

*I understand that a concert ticket is required to purchase for entry to watch my child/ward performance (watchout for the school annoucement).

*I agree that the school reserves the rights to reschedule or change the activities.

*With due diligence, I hereby agree to keep Dancepointe Academy & it's management and helpers of the event fully indemnified against all claims losses, damages or injuries arising from the event. I also agree that the school reserves the rights to reschedule and change Venue

*I agree to grant Dancepointe Academy full rights to take any photography and videography during the performance and to use any photos or videos of my child/ward* after the performance for the purpose of promoting Dance Education.

*I understand that the payment for the excursion is only available via PayNow, and the card payment option is currently inactive.
